【摘要】 目的:基于“筋喜柔,手法以柔和为贵”探索“柔和手法”对家兔骨骼肌慢性损伤的修复作用。为“柔和手法”治疗慢性软组织损伤提供客观的依据,进而为临床治疗慢性软组织损伤推拿手法的规范化提供可靠的依据。方法:选用40只健康成年家兔,30只采用机械冲击挫伤的造模方法建立慢性软组织损伤模型,随机分为“柔和手法”治疗组、“重刺激手法”治疗组、模型组,余下10只为正常组。柔和手法治疗组采用大鱼际揉法和指摩法治疗;重手法治疗组采用点按法和弹拨法治疗;模型组,不采取任何治疗,正常饲料喂养;正常组,正常饲料喂养。检测治疗前后家兔血清肌酸激酶(CK)、乳酸脱氢酶(LDH)含量水平。结果:“柔和手法”组与正常组比较,能明显降低血清中的CK、LDH的含量,(p<0.05);“重刺激手法”组与正常组比较,降低血清中的CK、LDH的含量不明显,(p>0.05);“柔和手法”组与“重刺激手法”组比较,在降低血清中CK、LDH的含量存在显著差异(p<0.05)。结论:“柔和手法”在家兔慢性软组织损伤修复过程中能够明显降低家兔血清中CK、LDH含量,疗效明显优于“重刺激手法”治疗组,为临床中慢性软组织损伤疾病的手法选择提供了客观依据。
【Abstract】Objective: based on the " the tendon feels like soft,and technique is better by soft" explore "soft" chronic injury of rabbit skeletal muscle repair. As "soft" provide objective basis for the treatment of chronic soft tissue injuries, thus for clinical treatment of chronic soft tissue injuries a reliable basis for the standardization of the massage. Method: chooses 40 healthy adult rabbits, use only 30 mechanical shock contusion model method to establish model of chronic soft tissue injuries, were randomly divided into the "soft" treatment group, "the stimulation technique" treatment group, model group, the remaining 10 for the normal group. Knead soft technique treatment group adopted the "yu ji" and refers to the method of treatment; Heavy technique treatment group treated by click and plucked method; Model group, do not take any treatment, normal feed; Normal group and normal feed. Test before and after treatment of rabbit serum creatine kinase (CK), lactate dehydrogenase (LDH) levels. Results: the "soft" group compared with normal group, can obviously reduce the content of CK, LDH in the serum, (p < 0.05); "Heavy stimulation technique" group compared with normal group, lower in the serum CK and LDH content is not obvious, (p > 0.05); "Soft" group compared with "heavy stimulation technique" group, in reducing the content of serum CK and LDH significant differences (p < 0.05).
Conclusion: the "soft" in the process of rabbit chronic soft tissue injuries repair can significantly reduce serum CK and LDH content on rabbit, "stimulation technique" is
obviously better than the effect of the treatment group, for chronic soft tissue injury in clinical diseases of technique selection provides an objective basis.

慢性软组织损伤[1]属临床中一种常见病、多发病、疑难病,严重影响着人们的身心健康甚至劳动能力,临床主要表现为慢性疼痛[2],慢性软组织损伤是长时期逐渐形成的,短期内不易观察到,待症状明显时,已经出现了器质性改变,因慢性软组织损伤发病广泛,治疗困难,以至于联合国卫生组织把它列为目前世界上三大类疑难病(癌症、心脑血管病、慢性软组织损伤)之一。笔者通过开展实验研究,依据客观的指标检测,来探索“柔和手法”治疗慢性软组织损伤的客观依据。
材料与方法
1、实验动物及分组:健康成年家兔均有昆明医科大学实验动物学部提供,
级别: 普通级别,数量:44只(4只预实验),体重:1.8千克——2.5千克,雌雄不限,重量分布均一。30只采用机械冲击挫伤的造模方法造模。随机分为“柔和手法”治疗组、“重刺激手法”治疗组、模型组,余下10只设立正常组,共四组。
2、实验方法:
2.1预实验:先选4只实验兔,其中2只实验兔参考朱利敏[3]等机械冲击挫伤的造模方法[13]制作兔慢性软组织损伤模型,其余2只正常饲养。连续3周后停止打击,无骨折及皮肤破裂,不做任何处理正常饲养20天后,检测血清中的CK、LDH,高于正常家兔血清中CK、LDH的含量,确定造模成功。
2.2动物模型制作:软组织损伤动物模型,参考朱利敏[3]等机械冲击挫伤的造模方法。实验前一天用8% 硫化钠脱毛剂除去一侧臀部及后腿被毛。实验时固定动物, 在左右后肢大腿外侧做好标记, 模型组用10 g 砝码从 1 m 高处坠落, 造成标记处局部软组织损伤, 打击点面积为 2cm×2cm。连续打击10下,连续3周后停止打击,无骨折及皮肤破裂,不做任何处理正常饲养20天,完成慢性软组织损伤兔模型的制备。
2.3分组:30只采用机械冲击挫伤的造模方法造模。随机分为柔和手法治疗组、重手法治疗组、模型组,余下10只设立正常组。共4组。
